Medical Services; No Medical Advice

Content on the Site is provided for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. Using the Site, taking an online assessment, or requesting an appointment does not create a patient-provider relationship.

Treatments and clinical services are provided in person at our clinic by licensed healthcare professionals, who exercise independent clinical judgment at all times. A licensed professional may determine that a requested treatment is not appropriate for you and may decline to provide it. Individual results vary, and no particular outcome is guaranteed.

Appointments and Cancellations

Appointments may be requested through the Site, by phone, or in person. An appointment request is not confirmed until you receive confirmation from us. Cancellation and rescheduling policies, including any applicable fees or deposit requirements, are communicated at the time of booking.

User Representations

By using the Site, you represent that:

All information you provide is accurate and complete
You are at least 18 years old or legally able to consent in your jurisdiction

Payments

You agree to pay all fees for products and services as presented at checkout, at the time of booking, or at the clinic. Payments may be processed by third-party vendors. Prices are subject to change without notice; the price in effect at the time of purchase applies.

Refunds

Payments for services that have been performed are non-refundable. Refunds for prepaid services or products that have not been provided are reviewed on a case-by-case basis; approved refunds are issued to the original payment method. Products that are damaged or incorrect on receipt will be replaced or refunded; contact us promptly at contact@rejuve.com.

Electronic Communications

Communications by Text Message and Email

By opting in, you consent to receive SMS and email communications related to your appointments, purchases, and optional marketing messages. You may opt out of SMS at any time by texting STOP. Upon sending “STOP,” we will confirm your unsubscribe status via SMS, and you will no longer receive SMS messages from us. Reply “HELP” for assistance, or contact contact@rejuve.com.

Messages and emails are not encrypted, and you acknowledge the associated risks. Carriers are not liable for delayed or undelivered messages. Message and data rates may apply, and message frequency varies. For questions about your text or data plan, contact your wireless provider.

Protected Health Information

Health information collected in connection with clinical services is handled in accordance with applicable law, including HIPAA where it applies, and as described in our Notice of Privacy Practices provided to patients at registration. The Site itself is not intended for the transmission of medical records; please do not submit sensitive health information through general contact forms.

Disclaimers

The Site and Services are provided “as is” without warranties of any kind. Rejuve Clinics does not guarantee uninterrupted access, error-free operation, or particular outcomes.

Limitation of Liability

To the fullest extent allowed by law, Rejuve Clinics and its affiliates will not be liable for indirect, incidental, or consequential damages arising from use of the Site or Services. Direct liability is limited to the amount paid by you for the applicable Services.

Indemnity

Governing Law

These Terms are governed by the laws of the State of California, without regard to conflicts of law. Any disputes shall be resolved in Los Angeles County, California.

Termination

Rejuve Clinics may suspend or terminate your access to the Site or refuse service for any reason, including violation of these Terms. The Disclaimers, Limitation of Liability, Indemnity, and Governing Law sections survive termination.

Assignment

You may not assign or transfer any of your rights or obligations under these Terms without prior written consent from Rejuve Clinics Sherman Oaks, Inc. Rejuve Clinics may assign or transfer its rights and obligations under these Terms, in whole or in part, without restriction, including in connection with a merger, acquisition, corporate reorganization, or sale of assets. These Terms bind and benefit the parties, their successors, and permitted assigns.

Entire Agreement

These Terms, together with our Privacy Policy and any other policies referenced herein, constitute the entire agreement between you and Rejuve Clinics Sherman Oaks, Inc.
